We encourage all students to participate in a brief masterclass, but if you choose not to participate, please check this box:

I agree to mail a check to Hannah Malone at 7380 FM 3405, Liberty Hill, TX 78642, and I understand that it must be received within one week of submitting this online registration form. The amount of the check should be $55 for the first student in the family, and $5 less for each subsequent family member (second $50, third $45, etc.).

If student is under 18, parents should check this box:
I authorize Hannah Malone or Tom, Chat, Aaron, or Ana Lerma to seek medical treatment for my above-mentioned son or daughter in the event of an emergency, after attempting to contact me for direction. I release these individuals, Amon and Joy Baker, and Milwood Baptist Church from liability for any accidents or damage which may arise out of my son or daughter's participation in Choir Camp.
